Fix all broken links found by the Robohelp link checker

Search for "Chapter" and insert links to chapters.

Search for "Official Comments" and insert links to any missing comments

Go through ALL of the interactive problem sets and check for broken links

See if I can fix the extra space problem in all of the links to sections everywhere.  This will require a carefully constructed search and replace across the entire project.

        It depends.  Sometimes the formatting was lost in the emails you sent,

        I could tell from the context.  Do you want the cases italicized throughout?
        YES. ALL CASE NAMES SHOULD BE ITALICIZED. JUST A CONVENTION. SORRY IF THAT WAS NOT CLEAR.
